Palliative Care

Growing old in the comfort of home is something many elderly persons hope for. As chronic illnesses become more common, more individuals hope to spend their final moments in a familiar and peaceful setting, surrounded by their loved ones.

“Tzu Chi Palliative Care” supports patients who have been diagnosed with a life expectancy of less than one year. Every moment of life is precious. Our care team is available around the clock and visits regularly to monitor the patient’s condition and help reduce the need for hospital admissions. We do our best to ease physical discomfort and emotional distress, and offer gentle companionship through sincere and mindful care.

We also work closely with families to understand the patient’s wishes and to walk together on this final journey with dignity and peace.

A Compassionate Journey into the Home

What We Do

Our palliative care supports patients with a smooth transition home from the hospital, followed by regular visits to monitor their condition, review medications, manage symptoms, and provide relief from pain and discomfort. We also work with partners such as The Compassionate Network https://www.tcn.sg/ to provide additional care, including art therapy and caregiver support, to accompany families in body, mind, and spirit.

My family member has been diagnosed with a terminal illness and is expected to have less than one year to live. How can we access Tzu Chi’s services?

Birth, ageing, sickness and death are an inevitable progression of life. At Tzu Chi, we believe no one should walk this journey alone. We are committed to accompanying individuals and their families as they approach the end of life, enabling them to do so in the comfort of home.

In Singapore, home-based healthcare and palliative care services are assessed and referred through the Agency for Integrated Care (AIC). As a charity organisation, Tzu Chi prioritises support for families from lower-income or vulnerable backgrounds, enabling patients to receive high-quality, long-term care under government subsidies.

At present, the majority of our home-based medical services are concentrated in the western region of Singapore, where our clinical teams are based.

Are patients eligible for support beyond medical care?

Tzu Chi collaborates with external partners to provide additional support for patients with broader needs, including financial assistance, social services, and social prescribing programmes. With our foundations in charity, medicine, and education, Tzu Chi is committed to a model of holistic and integrated care—supporting the patient through every stage, as a whole person, within the context of the whole family.

For lower-income households requiring further assistance, internal case referrals may be initiated. Following a home visit and needs assessment, we may offer tailored support such as living allowances, medical subsidies, psychosocial and spiritual care, and other appropriate forms of assistance based on the specific needs identified

Does Tzu Chi offer Advance Care Planning (ACP)?

Advance Care Planning (ACP) is a process in which individuals, while still in good health, discuss and document their preferences for future medical treatment and care. These conversations typically involve loved ones and medical professionals, and ensure that decisions made in times of serious illness reflect the patient’s values and wishes.

Tzu Chi has certified healthcare professionals trained to guide patients and their families through ACP. This process empowers individuals to make informed choices and ensures that medical care, when the time comes, is delivered in accordance with their personal values and intentions.
