In order to allow more elderly residents to spend their golden years in the comfort of their own homes, Tzu Chi has developed a patient-centred, integrated healthcare model. By bringing together its team of medical professionals and trained volunteers, the Foundation delivers coordinated and seamless Home Care Services to homebound patients, many of whom are elderly or living with chronic conditions.
As Singapore’s healthcare landscape continues to evolve towards inclusive and community-based care, the government has broadened the scope of Home Care Services and enhanced its subsidy schemes. In alignment with this national direction, Tzu Chi expanded its services in 2014 under the support of the Agency for Integrated Care (AIC). This initiative also realises the compassionate vision of Dharma Master Cheng Yen: to reach out to those who are suffering and unable to seek medical care on their own.

Through regular home visits, Tzu Chi’s healthcare teams gain a deeper understanding of each patient’s medical condition, home environment, and emotional wellbeing—enabling them to adjust care plans with empathy, attentiveness, and flexibility, tailored to each household’s unique needs.

To ensure the most appropriate and consistent care, all Home Care Services in Singapore are centrally assessed and referred through the Agency for Integrated Care (AIC). Tzu Chi is deeply committed to serving with compassion, prioritising support for patients from lower-income or vulnerable backgrounds, ensuring they receive consistent, quality care under government subsidy. Currently, our dedicated clinical teams are based in the western region of Singapore, and our services are primarily provided to households within this area. Tzu Chi believes in providing comprehensive support that extends beyond medical care. We collaborate closely with trusted partners to offer additional assistance such as financial aid, social services, and social prescribing programmes. Guided by our integrated mission in charity, medicine, and education, we are dedicated to offering holistic support that addresses the full spectrum of a patient’s and their family’s needs. For low-income households requiring further assistance, we are able to initiate internal case referrals. Following a compassionate home visit and assessment, we can offer tailored support such as living allowances, medical subsidies, psychosocial and spiritual care, and other appropriate assistance based on the specific needs identified.
Birth, ageing, illness, and death are natural stages of life. Tzu Chi is here to accompany you and your loved ones with compassion, ensuring peace and dignity in the comfort of home.
