Lakeside Family Medicine Clinic

Chronic conditions are long-term illnesses or symptoms that persist for three months or more, often changing in nature over time. Early detection and sustained management not only reduce healthcare costs but also help patients maintain a better quality of life.

In 2016, Tzu-Chi Foundation (Singapore) assumed management of the Lakeside Family Medicine Clinic (LFMC). Beyond general outpatient consultations, LFMC places a strong emphasis on the prevention and management of chronic diseases.

As part of Singapore’s Healthcare 2020 Masterplan, LFMC is integrated into the national Primary Care Network (PCN) and works closely with Ng Teng Fong General Hospital and Jurong Community Hospital to provide continuity of care for patients after discharge.

LFMC is also integrated with Tzu Chi’s in-house physiotherapy and home care services, allowing for a seamless care experience focused on holistic well-being.

  1. Free Eye Screenings for Seniors (vision issues, cataracts, and glaucoma)

    Comprehensive Eye Screening Services for Seniors (Free) – vision problems, cataract and glaucoma

    1. Dietary and Nutrition Counselling
    2. Nurse Consultations for Diabetes and Other Conditions
    3. Diabetic Retinal Photography (DRP) and Diabetic Foot Screening (DFS)
    4. Spirometry Services (Lung Function Test)
    5. Opportunistic Health Screening (for patients/caregivers)
    6. Laboratory Services
    7. Common Ailments (e.g., colds, flu, infections, minor injuries, digestive issues) \
    8. Treatment Procedures and Minor Surgery
      • Nebuliser therapy for acute asthma, wound dressing, suture removal, trigger finger injections
      • Ear irrigation and removal of earwax
      • Minor skin procedures for warts, skin tags, sebaceous cysts
    9. Pharmacy Services (Medications are dispensed at subsidised rates through NUHS)
    10. Holistic Support (Beyond medical treatment, we offer referrals for financial assistance, social services, and social prescribing, where applicable)

What is the difference between a Family Medicine Clinic and a GP Clinic?
Both Family Medicine Clinics (FMCs) and General Practitioner (GP) clinics provide general outpatient services. FMCs, however, are typically operated through partnerships between public healthcare systems and private providers. They offer more comprehensive and long-term care, particularly suited to the ongoing management of chronic conditions.
What services does a Family Medicine Clinic provide?

Since 2016, Tzu-Chi Foundation (Singapore) has managed LFMC in collaboration with JurongHealth. FMCs adopt a team-based care model, involving doctors, nurses, and allied health professionals to manage both acute and chronic medical conditions.

LFMC provides 23 chronic disease services under the Chronic Disease Management Programme (CDMP). Additional services include health screenings, diagnostic reports, dietary consultations, medical consultations, and subsidised medication dispensing.

Are government subsidies available?

Patients holding CHAS Blue or Orange cards, as well as those under the Pioneer Generation and Merdeka Generation, are eligible for healthcare subsidies.

Singaporeans enrolled in Healthier SG and managing chronic conditions may also use MediSave to pay for eligible treatment costs (subject to withdrawal limits).

Can patients receive support beyond medical care?

Tzu Chi collaborates with trusted external partners to assist patients with broader needs, including financial aid, social services, and community prescriptions.

Rooted in our mission across charity, medicine, and education, Tzu Chi aims to deliver comprehensive, person- and family-centred support. For households requiring additional help, internal referrals may be made. Following a home visit and needs assessment, we may offer assistance such as living subsidies, medical financial aid, emotional and spiritual care, or other appropriate support.
